Erogazione del servizio domiciliare

* Attuare interventi logopedici specifici, autonomi e complementari di natura tecnica, relazionale ed educativi in linea con le attività previste dal PAI.
* Garantire attività assistenziali orientate alle esigenze dei pazienti considerando età, genere, condizioni di salute, fragilità fisiche/psichiche e tenendo conto delle specificità religiose etniche e linguistiche.
* Garantire e promuove educazione sanitaria e terapeutica all'Assistito e al care giver
* Attuare le Linee Guide Internazionali e le Procedure definite da Medicasa
* Garantire il corretto utilizzo dei materiali e dispositivi messi a disposizione da Medicasa e corretto stoccaggio a casa dell'Assistito
* Assicurare l'identificazione e la pianificazione degli obiettivi assistenziali e verifica il monitoraggio degli outcome clinici.
* Garantire la corretta tenuta della documentazione sanitaria ed amministrativa utilizzando la Cartella Clinica informatizzata.
* Identificare segni e sintomi rispetto alla situazione clinica assistenziale e rilevare eventuali fattori di rischio
* Ottemperare alle responsabilità professionali di legge, agire secondo i principi propri dell'etica professionale e in maniera conforme al Codice Deontologico

2. Comunicazioni con la Centrale Operativa

* Partecipare alle riunioni di Team organizzate in Centrale Operativa
* Garantire la redazione delle relazioni sanitarie periodiche e se richieste dall'area sanitaria
* Partecipare alle riunioni indette dalla centrale operativa, dai distretti sanitari

3. Formazione

* Garantire il costante aggiornamento professionale con l'acquisizione dei crediti ECM come previsto dalla normativa nazionale.
* Garantire la partecipazione ai corsi di formazione organizzati da Medicasa

Competenza nella prevenzione, riabilitazione e cura dei disturbi del linguaggio
