Our community care service IC Home 24 cares for people in need of intensive care, ventilated and non-ventilated, with a high level of professionalism and social competence. Over 600 employees at 20 locations in Germany work with us.

Du leitest ein deutschlandweit agierendes Team von vier Case Managern und Case Managerinnen und berichtest an die Geschäftsführung der IC Home 24 GmbH. Du wirst nach außen im engen Kontakt zu den Kliniken, Behörden und Kostenträgern sowie nach innen im engen Austausch mit der Geschäftsführung stehen.
Du akquirierst neue Zuweiser und stellst die Belegungsrate in den Wohngemeinschaften sicher
Das Provider Management liegt in deiner Verantwortung
Die Kapazitäten für eine mögliche Aufnahme in unseren Wohngemeinschaften behältst du stets im Blick und stehst dafür mit den Regionalleitungen eng im Austausch
Der regelmäßige Kontakt zu Sozialdiensten, Krankenhäuser, Rehakliniken, Behörden und Kostenträgern zur Meldung freier Kapazitäten ist dein daily Business
Du weißt dein Netzwerk gut zu nutzen und knüpfst neue Kontakte mit Institutionen im medizinischen, therapeutischen und pflegerischen Bereich z.B. durch die Teilnahme an Arbeitskreisen, Kongressen und Meetings
Die Geschäftsführung der IC Home 24 GmbH hältst du durch Reportings stets auf dem laufenden
Auch mit allen anderen relevanten Stabsstellen stehst du zur kontinuierlichen Verbesserung der Unternehmensziele im engen Austausch
People for people | Your Carefluencer profile
Du bringst mehrere Jahre Berufserfahrung Vertireb und idealerweise im Case Management mit.
In den vergangenen Jahren konntest du dir bereits ein Netzwerk in der außerklinischen Intensivpflege aufbauen.
Auch Provider Erfahrung bringst du bereits mit.
Mitarbeiter zu motivieren und zu entwickeln ist dir ein großes Anliegen.
Du hast eine zielgerichtete und eigenverantwortliche Arbeitsweise.
Du bringst ein gutes betriebswirtschaftliches Wissen mit und handelst entsprechend den Kennzahlen.
Ein sehr gutes Verhandlungsgeschick gehört zu deinen Stärken.
